How to Qualify for Community Engagement Services:
To access these services through Heroes Helping Hand LLC, your child must be between the ages of 3 and 19, have a developmental disability or delay or be enrolled in or eligible for a Developmental Disabilities (DD) Waiver (Family & Individual Support, Community Living, or Building Independence) and have a recommendation for Community Engagement in their person-centered plan, coordinated by their Support Coordinator or Case Manager
